Sickness Certificates

The patient is responsible for self certification for the first six days of illness, using form SC1 for the self employed (which are available from reception) or SC2 for employees (which are available from your employer). We will not provide sickness certificates during this initial period. After this time you will need to make an appointment to see your doctor who will be able to issue a certificate. If the illness continues , or following hospital treatment, it may be possible to get a certificate without being seen by a doctor. Please telephone for advice.

Non NHS Examinations

Medical examinations for special purposes can be arranged by appointment. A fee is payable for all these examinations and our current price list is available in reception.
