How to register

It is easy to register with the Doctors at the Avenue Surgery. Simply come in and speak to a receptionist who will assist you through the process. It would be helpful if you could bring your current NHS medical card but this is not essential.

Change of Name/Address/Telephone Number

If you change your personal details than please make sure you advise us so that we can keep your records up to date.

NHS Treatment

Provided you have been resident in the UK for the last three months or you will be resident in the UK for the next three months you are entitled to free NHS treatment. If you do not meet either of these requirements and you cannot produce when requested the statutory European exemption forms then, under our NHS Contract, we are unable to provide free treatment and you will be charged for any services that we provide.

Unregistered Patients

If you have relatives or friends staying with you who become ill please do not hesitate to contact us. We will be happy to assist them. They will be treated free, as temporarily registered patients, provided that they meet the conditions for free NHS treatment. We will need details of their home address and the Doctor they are normally registered with. Others can be treated free of charges if their case is determined to be a medical emergency.

Languages

The Practice language is English and this is spoken by all our doctors and staff. We have no translators/interpreters available on site for any other language.
