Access to Medical Records

The Data Protection Act 1998 (Section 7) gives patients the right to apply for access to see their health records and to correct or delete incorrect information.

You can apply using an Application for Access to Medical Records form which is available from Reception if you wish. Please be aware that you will be asked to produce identification.

Each individual application is subject to an administration fee of £10.00.

The application must be made by the patient (being an adult or a child of 16 years or over), or a person with parental responsibility (for a child under 16 years).

The practice will check entitlement before processing the application. If copies of records are required, the Practice may charge up to £50 minus the £10 charge already paid depending on the amount of information available.

A response will be provided as soon as possible, usually within 21 working days. Where an application is declined, a reason will be given. In some strictly defined circumstances some parts of your record may be withheld.
