The General Data Protection Regulation (EU) 2016/679 (GDPR) gives patients the right to apply for access to see their health records and to correct or delete incorrect information.
You have the right to see what information we hold about you and to request a copy of this information.
If you would like a copy of the information we hold about you please contact our Practice Manager. We will provide this information free of charge however, we may in some limited and exceptional circumstances have to make an administrative charge for any extra copies or if the information requested is excessive, complex or repetitive.
We have one month to reply to you and give you the information that you require. We would ask, therefore, that any requests you make are in writing and it is made clear to us what and how much information you require. Where an application is declined, a reason will be given. In some strictly defined circumstances some parts of your record may be withheld.
The application must be made by the patient (being an adult or a child of 16 years or over), or a person with parental responsibility (for a child under 16 years).
