Access to Medical Records

The Data Protection Act 1998 (Section 7) gives patients the right to apply for access to see their health records and to correct or delete incorrect information.

Access to medical records must be in writing on a specific form (available from reception) and the request must be signed by the applicant.

Each individual application is subject to an administration fee of £10.00, which is non-refundable in the event that the application is declined.

The application must be made by the patient (being an adult or a child of 16 years or over), or a person with parental responsibility (for a child under 16 years).

The practice will check entitlement before processing the application. If copies of records are required, the Practice may charge up to £50 minus the £10 charge already paid depending on the amount of information available.

A response will be provided as soon as possible, and in any event, within 21 days. Where an application is declined, a reason will be given. In some strictly defined circumstances some parts of your record may be withheld.
